What is a Care Manager?

A care manager is a professional who specializes in assisting with care assessments and care coordination. Our care managers have a minimum of a Bachelor’s degree or equivalent training in gerontology, social work, nursing, counseling, or human services.

Our care managers are:

·     Experienced nurses and/or social workers, providing clients with objective needs assessments and service recommendations;

·     Expert Resources providing support by answering your questions and concerns based on years of experience in care management;

·     Guides for clients, providing assistance through the maze of health care options and alternatives;

·     Personal advisors, working with clients to develop, coordinate and implement appropriate care plans to meet care needs and reduce uncertainty and stress;

·     Care advocates, addressing care concerns and acting on the client’s behalf;

·     Community resource specialists, offering clients accurate information on service providers, housing options, agencies and programs;

·     Liaisons between clients and their families and caregivers, providing peace of mind by ensuring that loved ones receive the highest quality of care.
